All fees due at the time of service. We will be happy to provide you with appropriate billing codes that you can submit to your insurance for reimbursement.
Some insurances with whom we used to participate may still list us on their websites. We have asked them to update their websites.
In-Network
We will bill your insurance directly. Reimbursement from your insurance company is not guaranteed. Coverage depends on your individual policy; any deductible amount, copays, co-insurance or other expenses not covered by the insurance plan are the responsibility of the patient/family. Procedure codes most often billed to your insurance company include 90791, 96132, 96133, 96136, 96137, and in some cases 96138 and 96139. You are encouraged to contact your insurance carrier prior to your appointment to obtain/understand coverage information regarding your policy and determine coverage for those procedure codes as provided under your specific policy. We typically collect deductible amounts on the day of the evaluation.
Out of Network
You will be provided with an itemized invoice that will include procedure and diagnostic codes that can be shared with your insurance company for reimbursement. Reimbursement is not guaranteed, and it is the patient/family’s responsibility to check with their insurance company to determine coverage.
